Telephone Assisted Immediate Adult Patient Resuscitation by Emergency Medical Dispatcher

This bachelor's thesis deals with Telephone Assisted Immediate Adult Patient Resuscitation by Emergency Medical Dispatcher. This thesis was divided into two parts. The first part was theoretical and the second part was practical. The main focus of the theoretical part was on medical emergency service and medical operations centre. Heart anatomy and cardiac arrest are also mentioned in this thesis, as well as problematics of an urgent resuscitation, chain of survival and telephone-assisted immediate resuscitation. The second part was the research part. Two research questions and two objectives were established in this part. First objective was to map knowledge of dispatchers of a medical operations centre in telephone-assisted immediate resuscitation. Second objective was to map how dispatchers of a medical operations centre proceed in provision of telephone-assisted immediate resuscitation. Semi-structured interview method was performed in the practical part of this thesis. Sample consisted of 10 dispatchers of the Regional medical operations centre of the South Bohemian Region who were asked 27 questions. For the sake of clarity, collected data were divided into 14 categories and processed into tables. Work objectives were fulfilled and research questions were answered. Research showed that dispatchers have stronger theoretical knowledge of telephone-assisted immediate resuscitation management of an adult patient, rather than of a pediatric patient. Research also mapped out the procedure of telephone-assisted immediate resuscitation of each individual responder. Results are rather positive.
The Work of an Operator of Emergency Medical Service

This bachelor thesis mostly dealt with the activity of medical controllers as well as the issues of medical operative center in general. Many people don´t know what it is all in all. It is only undemanding sitting at the table and answering the phone for most people. I would like to point out the facts related to this profession and refuted this claim of the majority. This thesis mentions the medical operation center, its history in the Czech Republic, and it is interested in its tasks, layout and equipment. There are also mentioned the activities of emergency medical service-controllers themselves, including their education or criminal responsibility. Emphasis is also placed on the stress on medical service-controllers in the performance of this profession. A large part of the work is devoted to taking the emergency call, telephone-assisted emergency resuscitation and telephone-assisted first aid. Mapping the theoretical knowledge of medical service-controllers and finding out the individual procedures for providing the telephone-assisted resuscitation of both, adults and children, was one of the aims of this bachelor thesis. Another goal was to determine the satisfaction of service-controllers within the performance of their profession. There was performed a qualitative research using the interview technique. The interview included knowledge and satisfaction with this profession as well as the issues of telephone-assisted resuscitation of adults and children. According to the results of this survey there was found that the medical service-controllers are not sufficiently prepared to perform this profession immediately after the graduating their studies. They use the same or very similar procedures within their telephone-assisted resuscitation of an adult or child. The survey also shows they are satisfied with their job.
The Awareness of South Bohemian Public about Cardiopulmonary Resuscitation

INTRODUCTION: One of the most frequent causes of sudden death in Europe is a sudden cardiac arrest. First above all providing urgent cardio-pulmonary resuscitation (CPR) is necessary for rescue of life. One of preconditions of successful resuscitation is its early start and knowledge of basic techniques. The level of knowledge of urgent resuscitation techniques, its proper performance, skillfulness and willingness of non-professional rescuers can decide about next life of people affected by sudden cardiac arrest. Every minute that the resuscitation is not performed a chance of survival of people affected with sudden cardiac arrest decreases by 10 - 12%. GOALS AND METHODOLOGY: The main research problem was an evaluation of the level of knowledge of techniques of cardio-pulmonary resuscitation with public in South Bohemia. The next goal was to map the interest of the public in South Bohemia in education in CPR. For working out of the empirical part the method of quantitative research was chosen in the form of a questionnaire which I made. The questionnaire contained 35 questions. Dispatcher-assisted cardiopulmonary resuscitation

The main task of providing dispatcher-assisted first aid is to guide bystanders to giving the first aid to the victiim directly in the situation where the incident happened according to the possibilities and conditions. It's one of the basic roles of medical dispatch centre and each dispatcher working on dispatch centre has to manage this role. These basic instructions should be provide in many different situations. One of this situation is pre-hospital cardiac arrest. When the dispatcher detects cardiac arrest during the emergency call, he/she should start providing dispatcher-assisted cardiopulmonary resuscitation (DA-CPR) immediately. It means telephone assistance and guidance of bystander to perform cardiopulmonary resuscitation on the place of incident. The main aim of the research is to find out, whether the dispatchers are able to detect cardiac arrest in time and respond to it appropriately. It means iniciate the bystander to begin cardiopulmonary resuscitation, motivate him/her, instruct him/her correctly and easily, calm the situation and guide the bystander till the ambulance arrives. The role of the medical operations center, a provider of emergency medical services

Bachelor's thesis deals with the medical operating center and its activities. A lot of people have no idea what a medical operating centre actually is. My knowledge about it was also scarce before the beginning of my studies. Therefore I would like to outline the problems, tasks and also the demanding character of the work of operators. That is why this thesis is aimed at examining the tasks of the medical operating centre. Everybody probably knows the term medical rescue service and imagines immediately a yellow vehicle with reflective strips and blue lights. Hardly anybody realizes, however, that the medical rescue service needs the medical operating centre to be functional. It may be also called a control centre. This centre is an inseparable part of the medical service and also of the integrated rescue system. Specialized medical workers, called operators or controllers, are employed here. These workers, among others, receive continuously emergency calls, evaluate them and pass them on to exit groups, and, last but not least, assist the caller with giving first aid. All these problems are the subject of the theoretical part of the thesis. Two research issues were determined for the purpose of the research part of the thesis. The first one deals with all tasks ordered by legislation being used by the operating centre and the second research issue is aimed at the problems of urgent basic life support assisted by phone, namely if this basic life support assisted by phone is utilized always when unconsciousness is indicated. These issues were dealt with by means of the qualitative research method consisting in the secondary data analysis. The research set constituted the regional operating centre of the South Moravian Region. Specifically, documentation of emergency medical services was used operator record and audio record of emergency call. This documentation is a part of each emergency call. By means of the secondary analysis the data from 20 emergency calls were obtained. Emergency calls were chosen by simple random choice. The sample of these 20 emergency calls was chosen from the period of the year 2014. Because of protection of personal data no names are given in the thesis, only the initials. For the same reason no actual addresses are used in the research. In the discussion there are given all tasks ordered by legislation and recommended procedures from professional sources. This information is subsequently compared with the obtained data. The results in the case of the first research issue indicated that some tasks ordered by legislation are not used too often. Some tasks within the framework of the small sample being examined are not used at all and, on the contrary, they are used in the receiving of each emergency call. The tasks that were not used at all were three altogether. The results in the case of the second research issue demonstrated that indication of unconsciousness is not always necessarily the reason for using urgent basic life support assisted by phone. It is important that also other aspects are taken into account, not only consciousness of the patient. Of ten emergency calls concerning unconsciousness being examined this urgent basic life support assisted by phone was used in five of them. It was found out that utilization of the medical operations center tasks is individual in each emergency call. From the point of view of the total activity of the centre it could be declared, however, that the centre uses most of the tasks ordered by legislation. This bachelor's thesis can be used as a material extending the knowledge of both non-professional public and medical personnel concerning the activities of medical operating centre. It can be a stepping stone for further research concerning medical operations center.
The telephone assisted urgent resuscitation by means of despetcher of medical rescue service

Any time soon each of us can become a witness to a situation when someone else´s life is at immediate risk. Some people naturally remain calm and without major hesitations start saving the human life. However, there are fewer such people than those who, as a result of insufficient knowledge about urgent resuscitation, may endanger the chance of survival of others, because they are not able to provide basic first aid. Operation centers of medical emergency services know how to help such people to respond properly in emergency situations. The employed method is called the dispatcher-assisted telephone-guided cardiopulmonary resuscitation (the Czech acronym is TANR), which means that a worker of the operation center uses, in a life-threatening situation, a telephone to guide the person who can save life of a close person or even life of a stranger. This bachelor thesis in its theoretical part describes activities of the medical emergency service, the main tasks and content of work performed by the operating centers and its personnel. It also defines the term ``dispatcher-assisted telephone-guided cardiopulmonary resuscitation{\crqq}. The thesis describes methods of urgent resuscitation and in which situations they should be performed. The survey for this thesis was based on questionnaires distributed to dispatchers of medical emergency services in the Moravian-Silesian and Zlín regions. The questionnaire contained questions concerning their knowledge of and experience with the provision of TANR. Based on the survey I have come to the conclusion that TANR represents for the workers of operating centers a method to assist people in difficult, life-threatening situations.
